Turning 18 or leaving for college: four documents to sign

The day your child turns 18, their school records become theirs and doctors generally talk only to them. These four documents let them choose to keep you in the loop if something happens.

Good to know

  • Your child decides. Have the conversation first; it's their information.
  • If the school is in another state, check that state's rules for health care proxies. States' forms and requirements differ.
  • Sign before move-in, save PDFs on both phones, and give the health center a copy.

We notarize at your home, or online by video if your child is already on campus. Summer is our busiest time for these, so book early. Book a notary

Questions

Can a parent see a college student's medical records?

Generally not once the student is 18, unless the student signs a HIPAA authorization or names the parent in a health care proxy.

Does a HIPAA authorization need to be notarized?

No. Federal rules don't require a notary or witness, though a provider may have its own form.

Can we sign if my child is already at school?

Yes. A New Jersey notary can notarize by video for many documents, or the student can use a notary near campus.
