The short version
An acknowledgment is where you tell the notary that the signature on the document is yours and that you signed it willingly. A jurat is where you swear or affirm that what the document says is true, and you sign in front of the notary. One is about your signature. The other is about the truth of the contents.
Both require that you appear before the notary and show acceptable identification. The wording on the certificate usually tells you which act applies, such as 'acknowledged before me' or 'subscribed and sworn to before me'.
How an acknowledgment unfolds
The notary verifies your identity, then asks whether you signed the document for the purposes it states and whether you did so freely. You can sign before the appointment, since the key step is that you confirm the signature as your own. Deeds, powers of attorney and many contracts carry acknowledgments.
The notary then completes the certificate with the date, the place, your name and their signature and seal. The notary does not read the document for accuracy.
How a jurat unfolds
For a jurat, you must sign in the notary's presence, so do not sign ahead of time. You may raise a hand or rest it on a book if you prefer, and then you swear or affirm that the statements are true. An affirmation uses no religious language and carries the same weight.
Affidavits, sworn statements and many application forms use jurats. If you cannot truthfully swear to the content, you should not sign. Do not ask the notary to review the facts; they only administer the oath.
Who picks the act
The signer picks, or the agency or person who will receive the document picks. The notary cannot choose for you and cannot explain legal effects. If your document has no certificate, the notary will ask which act you need and can attach a loose certificate page.
If the certificate printed on the page does not match what the receiving office asked for, ask the office before you meet. A notary can often cross over to a correct form if you tell them what you need, but they cannot guess.
Common mix-ups
People sometimes sign a sworn statement ahead of time and then expect the notary to jurat it. The notary will ask you to sign again or to switch to an acknowledgment if the form allows. Another mix-up is bringing a document with an out-of-state certificate. Some offices accept it, others want state-specific wording.

Which is more serious?
A jurat involves an oath, so false statements can carry penalties. An acknowledgment confirms signing. Neither is automatically better; each fits different documents.
Can I take an oath over a video call?
Where remote online notarization is allowed, a signer may be sworn over video. Rules depend on the document and the receiving office.
What if my document has no notary block?
The notary can attach a separate certificate page once you tell them which act the receiving party requires.
