Caregiver Authorization for School and Doctor Visits

Purpose of the letter

The letter is signed by the parent or legal guardian and names the caregiver. It lists the tasks permitted, such as enrolling the child in school, signing permission slips, obtaining routine medical care and talking with teachers. It does not give the caregiver permanent custody. Courts handle that.

Key details to include

Include the child's full name and date of birth, the parent's and caregiver's names and addresses, a start and end date, and a description of the authority granted. Add contact numbers for the parent. Add insurance details if the caregiver will take the child to the doctor.

The notary does not write this content. If a school or clinic has its own form, use it first.

The notary's checks

The parent signing must appear with a government ID. The notary compares the name to the document and confirms that the signer is acting willingly. Typically the certificate is an acknowledgment. If the parent is out of the area, remote online notarization may be possible if the receiving office allows it.

The caregiver does not need to be present unless the form requires their signature too.

Where it is used

Schools may accept the letter for enrollment or for pick-up permissions. Clinics may ask for it before treating a minor without a parent. Insurers and pharmacies may have their own rules. Call each office to ask if a letter is enough or if they want a specific form.

Have several signed originals prepared when multiple offices need one. Each notarization is separate, so request an extra act per original.

Review and revoke

Put an end date on the letter and renew it when needed. A parent can withdraw the authority at any time by telling the caregiver and the offices that hold a copy. If the arrangement will last long, speak with an attorney about guardianship options.

A notary can help with the signing but cannot say which arrangement is better for your family.

Scenario: a grandmother caring for a grandson during a deployment

A parent is stationed away for months. The grandmother takes the child to school and the pediatrician. The parent drafts a letter naming her, the dates, and what she may do, and signs it in front of a notary. If the parent is not near a notary, a remote session may serve.

The school office keeps a copy and updates the pick-up list. The pediatrician keeps a copy in the chart. The grandmother carries the original and the child's insurance card. If the parent can still be reached, the letter includes a phone number and email.

Some benefits, such as public assistance or health coverage, have their own forms and may not accept a letter. Ask each agency. If the arrangement may last for years, or if the parent cannot be reached at all, an attorney can explain court options.

Mistakes that get documents rejected

The usual problem is assuming one letter works everywhere. Schools and clinics have their own rules, and some require their own form instead of a letter. Other problems are vague wording about what the caregiver may do, no start or end date, and missing contact information for the parents.

Before you write anything, ask the school or provider what they accept. A notary can confirm the signature but cannot say whether the document meets their requirements.

Checklist before you book

Ask the receiving office what it requires. Name the child and the caregiver. State exactly what is authorized and for how long. Include parent contact details. Sign in front of the notary and keep copies for each office.
