What a living will covers
It generally covers choices such as life support, feeding tubes, resuscitation and pain relief. It may be a stand-alone page or part of a combined advance directive. Doctors and hospitals each have familiar forms, and an attorney can adapt one to your situation.
A notary cannot advise on these choices. Talk with your doctor and family before the signing so no one is surprised.
Witnesses and notaries
Requirements differ. Some forms need two adult witnesses, some accept a notary alone, and some ask for both. Witnesses are often barred from being the agent, heirs or healthcare staff treating the person. Follow the form's list.
If you need witnesses, bring neutral adults who can show ID. A notary's service can supply witnesses on request.
The signing sequence
The signer shows ID, states their name and confirms they are signing willingly. The signer signs, the witnesses sign in the signer's presence, and the notary completes the acknowledgment. The notary then records the act in the journal.
Take your time with each page. If you want to change a choice, start a clean copy rather than scribbling on the signed page.
Where copies should go
Give copies to your doctor, your healthcare agent and your family. Some hospitals scan advance directives into the patient file. Keep the original in an easy-to-find place at home. Tell people where it is.
Review it after major events such as a diagnosis, a move or a divorce. A signed update replaces the older copy only if you destroy or clearly revoke the earlier one.
Common problems
Typical issues include a witness who is a relative or beneficiary, a missing initial on one page, and a signer whose ID is expired. An expired ID can usually not be used. Check dates before the appointment.

Wording that people often get wrong
General phrases like 'no heroic measures' can mean different things to different readers. Doctors prefer clear statements about specific situations, such as permanent unconsciousness or a terminal condition. Your attorney or doctor can help phrase your wishes so they are easy to follow.
Another gap is mixing the living will with the healthcare proxy in a way that creates conflict. If the two documents say different things, hospital staff may not know which to follow. Have them prepared together, and read them side by side before signing.
Finally, check the date and the printed names. A witness who signs with a nickname instead of the name printed under the line can cause questions. The notary can ask each person to print and sign as they are known, but cannot revise the text for you.
Mistakes that get documents rejected
Using ineligible witnesses, signing in the wrong order and not distributing copies are the common problems. Requirements differ by state and by form, so follow the form's own instructions.
Make sure your doctor and your healthcare agent have copies, because a document nobody can find does not help in an emergency.
