What the notary is and is not doing
The notary identifies the principal, confirms the principal appears willing and aware, and completes an acknowledgment. The notary does not decide who should be agent, explain powers or confirm that the document is valid under state law. Those questions belong to an attorney.
If the principal seems confused or pressured, the notary may pause or decline. This protects both the principal and the notary.
Documents and people to bring
Bring the original document, unsigned, plus a valid photo ID for the principal. If the form calls for witnesses, arrange them ahead of time. Some forms also want the agent to sign an acceptance page. The agent does not have to attend, but if the agent signs, that signature may need its own acknowledgment.
Check the printed names carefully. The principal's name on the form should match the ID, including middle names or suffixes.
Typical sequence
The notary reviews the ID, asks the principal to confirm that they are signing willingly, and watches as the principal signs and dates the document. If witnesses are needed, they sign too, and the notary then fills the acknowledgment. Every page that asks for initials should be initialed.
Expect the notary to ask whether any pages are missing, and whether any blank spaces remain. A form with fill-in blanks left empty may be questioned.
Capacity and independence
A notary does not diagnose. They simply observe whether the signer can say who they are, what they are signing, and who is helping them. If a family member answers every question, expect a polite request for a private moment with the signer.
If the signer has had a recent illness or is in a care setting, it helps to schedule at a time of day when the signer is rested and alert.
After it is signed
Store the original in a safe place and give copies to the agent and, where useful, to banks. Some institutions have their own power of attorney forms or review periods, so call ahead. If the document will be used for real estate, recording may require its own formalities.

Questions to settle before the appointment
Choosing an agent is the heart of this document, and the notary cannot help with it. Many people pick a spouse, an adult child or a trusted friend, and name a successor in case the first choice cannot serve. Talk to the people you name so they are not surprised.
Ask your attorney about when the powers begin. Some documents take effect at once, while others wait for a triggering event. Also ask about gifting powers, authority over retirement accounts and whether the agent should be paid or must keep records.
Banks and brokerages sometimes prefer their own forms. If you have a main account somewhere, call and ask whether they accept an outside power of attorney or want theirs signed as well. Completing both on the same day with the same notary can save a second appointment, and each one will carry its own notarial certificate and journal entry.
A typical appointment, step by step
The principal and the notary meet, the principal shows ID and confirms they understand the document and are signing willingly. The principal signs, then the notary completes the certificate. If witnesses are needed by state law or the form, they sign as well. The notary does not review the contents or decide whether the document is right for the signer.
Checklist before you book
Bring the complete document. Bring valid ID. Bring witnesses if required. Make copies. Tell the agent where the original is kept.
Common questions
Does the agent need to be present?
Not usually. Only the principal's signature is acknowledged, unless the form also asks the agent to sign.
Is a durable power of attorney the same as a healthcare proxy?
No. A durable power of attorney is generally about finances and property. Medical decisions use a separate document.
What if the principal can no longer sign?
Speak with an attorney right away. A notary cannot take a signature from someone who cannot understand or take part.
