What the notary compares
The notary looks at your ID and compares the name on the ID with the name signed on the document. When a petition shows both your current name and the proposed name, the notary relies on the current name, since that is on your ID. Do not sign with the proposed name until the court approves it.
If your ID shows a former name, tell the notary. They can note it but cannot decide which name is correct for the court.
Sworn statements
Many petitions include a jurat, which means you swear the contents are true. Read the form fully before the appointment. Dates of birth, places of birth and prior names must be accurate. Errors can cause the court to reject the filing.
If you need to list previous legal names, bring a list so you can check it against the form.
Other related notarizations
Parents changing a child's name may need consents from both parents, which each require ID and signature. Some courts also ask for a notarized statement that the petitioner is not changing a name to avoid debts or legal duties.
After the order, an agency may ask for a notarized copy statement. Notaries usually cannot certify court records themselves, so you may need the clerk to provide a certified copy.

Check that your ID is current, bring every adult signer, and confirm if the court wants originals or copies. Keep the finished forms in order of filing.

A rough order of operations
First, the petition is prepared and sworn, then filed with the court along with fees and any publication or hearing steps the court requires. After a judge signs the order, you request certified copies from the clerk.
Second, update the records that unlock the others. Many people start with Social Security, then the motor vehicle agency, then banks, employers, insurers and passport. Each has its own form and ID rules. Some will want the certified court order, and a regular photocopy will not do.
Third, keep a master list of everywhere your name appears: utilities, voter registration, professional licenses, loan accounts, subscriptions. Work down the list over the following weeks. Notarization is only needed for a few of these steps, but the same careful name matching applies to all of them, so use your new name consistently once the order is final.
