When the form is used
The form is for the parent who will not be present when the application is submitted. That parent signs the statement of consent before a notary. The other parent, or the person applying, brings the notarized form along with the child's application.
Special rules apply for children over a certain age and for cases of sole custody. Read the instructions.
Matching the application
The statement should identify the child, and there is often a line to confirm which application it relates to. Many applicants attach a photocopy of the absent parent's ID. The notary does not do that part; the form tells you what to attach.
Complete the form only as instructed. Do not sign it ahead of the notary.
At the appointment
The consenting parent brings valid photo ID, appears in person and signs. The notary completes the notarial block printed on the form. Many notaries use a seal that includes the commission expiration, which the agency can read. The notary does not collect the application or fee.
If the signing parent is in another country, a consulate or a remote online notary may handle it.
Common reasons for returns
Typical reasons are a form signed before meeting the notary, an unreadable seal, a missing date or missing attachment of a parent's ID copy. Another is using an outdated version of the form.

Step list for families
First, download the current form from the official government site and read the instructions. Second, complete the child's application but do not sign it until you are at the acceptance facility if the instructions say so. Third, have the absent parent complete the consent statement and sign before a notary.
Fourth, attach a photocopy of the front and back of the absent parent's ID as instructed. Fifth, submit all items together with proof of citizenship and the child's photo. Keep copies of what you send.
If the absent parent is not able to sign for any reason, the instructions describe other routes, including statements explaining the circumstances. That is a legal and agency matter, not a notary one. Government processing times change, so check the agency's page for updates before you plan travel.
If your child is near the age at which different rules apply, check the agency's current guidance before you book travel. Processing times, fees and which kids need the form can change, and the instructions at the official site are the final word. Allow a buffer for mailing time and a second notary appointment in case anything on the form needs a correction.
