FAQ
Common questions
How soon do I need to use it?
The State Department asks that the form be submitted within 90 days after it's signed and notarized, so book close to the passport appointment.
What do I bring?
The completed form, unsigned, and your government photo ID. We'll give you a copy of the ID for the application.
I'm overseas. Can I still use you?
Often, through remote online notarization. Some countries require you to use a U.S. embassy or consulate instead, so check the State Department's instructions for where you are.
