Online notarization by document
Medical and school consent forms, signed by a parent on video
A school, camp, team or doctor's office wants a parent's signature notarized, and the parent is at work, on a trip or in another state. A short video session can cover it if the program accepts an electronic notarization.
Starting at $40 per session, online, no travel · quote confirmed before you book · fees
Matthew Coleman, NJ Notary Public and NNA Certified Loan Signing Agent. Cranford, NJ. Fee confirmed before your session. Card charged only after it is complete. We aim to confirm within 15 minutes, 7am to 9pm.
What this document is
These are forms in which a parent or legal guardian gives permission for something involving a child: a relative or caregiver consenting to medical treatment while the parent is away, a school enrollment or residency affidavit, a camp or sports medical release, or permission for someone else to pick the child up. They usually come from the school, program or provider, which also decides whether a notary is needed.
What the consent should cover, and for how long, is between the family and the program, or the family's attorney. The notary identifies the parent and notarizes the parent's signature.
Does online notarization usually work for it?
Because the person signing is usually the parent, not the child, and the forms are short, these are well suited to an online session when the program accepts a PDF. Ask the school, camp or provider before you book:
- Does this form need to be notarized, and will you accept an electronically notarized PDF?
- Do you need both parents' signatures, or one? Do you need a copy of a custody or guardianship order?
- Does the consent have to cover specific dates, a named caregiver or particular activities?
- Is a paper original required at drop-off or check-in?
Pitfalls to avoid
The child doesn't sign. The parent or guardian signs and shows ID. The child doesn't need to be on the call.
Two parents, two places. If the program wants both parents, each needs their own ID check. They can join the same session from separate devices, or book separately.
Blank dates and names. Fill in travel dates, the caregiver's name and the child's details before you upload. The notary can't complete them.
Signing as guardian. If you sign as a legal guardian rather than a parent, the program may ask for proof. Ask what it needs; the notary only identifies you.
A parent outside the U.S. New Jersey adds conditions when the signer is abroad (see the box below), including that signing is not prohibited where the parent is. Tell the desk the country when you book.
Readiness checklist
- A valid, unexpired government-issued photo ID for each signer. We ask for a current ID for online sessions; if yours has expired, call the desk before you book.
- A computer, tablet or phone with a working camera and microphone.
- A stable internet connection, somewhere quiet enough to hear and be heard.
- The complete document as a PDF, with every page and no blanks, not yet signed.
- A yes from the receiving party that it accepts an electronic notarization.
- Your location: anywhere in the United States is fine. Outside the U.S., the document has to meet New Jersey's conditions (see below).
- The program's own form, if it has one, rather than a generic template.
- The child's full name and dates exactly as the program wants them.
Signing from outside the U.S.? New Jersey requires that the record be filed with or relate to a matter before a public official, court, governmental entity or other entity subject to U.S. jurisdiction, or involve property in the U.S. or a transaction substantially connected with the U.S.; and that signing it is not prohibited by the country you are in. See signing from outside the U.S.
How the session works
- Check the program's form. Use the form the school, camp or provider gave you. Fill in everything except the signature.
- Book and upload. Upload it unsigned. If two parents are signing, say so.
- Identity steps. Each parent or guardian completes the ID check.
- Sign on video. The notary confirms who is signing; each parent signs on screen and the notary completes the certificate.
- Send it to the program. Email or upload the notarized PDF the way the program asked, and keep a copy for trips.
Your notary is a commissioned New Jersey notary working from New Jersey. The video session is recorded and the recording is kept for at least 10 years, as New Jersey requires.

FAQ
Medical and school consent forms: common questions
Does my child need to be on the call?
No. The parent or guardian is the signer, so the child doesn't need to appear or show ID.
Can both parents sign from different places?
Yes. Each parent joins from their own device and completes their own ID check, in the same session or separately.
Will the school accept an online notarization?
Each school and program decides. Ask them before you book.
Can you tell me what my consent form should say?
No. We notarize; we don't draft or explain documents. The program usually supplies its form; your attorney can advise on anything beyond it.
Is a passport consent form the same thing?
No. The passport consent for a child under 16 is a federal form with its own instructions. See our passport consent page.
